I have updated the visualisations of the RF2 ref set and other rmeta-data at http://www.openehr.org/wiki/display/term/SNOMED+CT+RF2 which may help people get their heads around it.

Regarding the clinical metadata elements you mention, these are not defined as part of RF2, but it should be possible to represent them using RF2 as it was designed to be an extensible format.
